**PLEASE NOTE PTS WILL BE PREPARED IN ACCORDANCE TO HOCKEY CANADA AND OHF GUIDELINES** If you have more than one child who needs a form(s), please fill out a separate request form for each child.
**Please allow 1-2 business days for your PTS Request to be processed. Once the form has been signed by Hamilton Steel, we will email you a copy of the PTS form via the email address you provide on the submission form. You must print and sign the approved PTS and bring it with you to your tryout/training camp.
